General Information

A passport is necessary when traveling out of the jurisdiction of the United States. The Passport Department is located in the Clerk's Recording Office at 161 E. Jefferson Street (across the street from the Courthouse.) **Please note the mailing address is: 20 N. Main Street, Rm #215, Brooksville, FL 34601.) The telephone number is (352) 540-6382. Office hours are 8:00 AM to 5:00 PM.

To obtain a passport, you must submit the following:

  1. Certified copy of birth certificate with a raised or colored seal and a filing date within one year of birth. No hospital certificates, birth notices, birth registrations or announcements are acceptable.

  2. Two recent passport photos are required for each passport applied for.
  3. Application Fee - CHECK OR MONEY ORDER - payable to U.S. DEPARTMENT OF STATE - $75.00 per adult, $60.00 ages 15 and under . Please note: for faster processing, you may request expedited services. The additional fee is $60.
  4. Clerk Processing Fee made payable to CLERK OF CIRCUIT COURT for $25.00. Cash or check is acceptable.
  5. Identification - A valid Driver’s License or State ID card.
  6. When completing the DS-11 (Application for Passport) answer questions 1-22 ONLY. DO NOT SIGN THE APPLICATION! IT MUST BE SIGNED UNDER OATH IN FRONT OF THE COURT CLERK.
  7. Children 16 years old and over do not have to be accompanied by their parents to the Clerk’s Office to apply for a passport if they have a valid Driver’s License. Learner’s permit cannot be used for identification purposes. If you do not have a valid Driver’s License, your parents must accompany you to the Clerk’s Office and your parents’ Driver’s Licenses will be used for identification purposes.
  8. Children under 16 years old must be accompanied by their parents. Both parent’s signatures are required on the application and both parents’ Driver’s License will be used for identification purposes. If the child’s last name is not the same as the parents, PLEASE LET US KNOW. A certified copy of the Marriage Certificate or divorce papers may be required to show a link between both names for proof of citizenship.

The above items must be submitted with your application and will be returned along with your passport.
